| Statement
of Facts Prepare and provide us a chronological statement of facts based upon your initial client intake. Include client's signed statement of facts. Identify all potential defendants, and working theories regarding their liability, individually. Summary should include the specialties of each potential defendant and indicate the relevant statute of limitations. |

| Medical
Records Forward a complete set of medical records relevant to the permanent residual injuries suffered by the plaintiff. The records should include the hospital admission sheet, discharge summary, admission history and physical, physician's progress notes, consultation reports, doctor's orders, operative reports and all nurses notes, plus the office chart for the patient from the defendant(s) and subsequent treating physician(s). |

| Tissue,
Blood Slides and Photographs Do not submit tissue or blood slides unless requested. Send relevant photos. We suggest that they be dated, using masking tape on the reverse side with the date noted to preserve them as potential exhibits at the trial. |

| X-rays Send copies of pertinent x-rays. Retain the originals in your file. If you cannot determine the copy from the original, you have a proper copy. Be certain that your client's name, date, and test performed, is legible on the copy. In fracture cases, send films of the acute injury, the follow-up x-rays ordinarily obtained 8-10 days after the fracture is casted and x-rays taken at the completion of therapy. In contrast studies, e.g., angiograms, venograms, intravenous pyelograms, barium studies, MR or CAT scans, forward all available films. |

| Autopsy If an autopsy was performed, obtain the file diagnosis page, bacteriological report, the toxicology laboratory report, the microscopic description and gross anatomic findings, etc. |
|||||||||||
| Depositions If the case has been filed, forward copies of all discovery depositions that relate to the medical facts or opinions. |
